Do you need 1 or 2 dd form 1574 for army aviation parts turn in?

For aviation parts turn in, is it a REQUIREMENT for the part to have TWO yellow tags? I know two are required for green and red tags, but I thought one was necessary for yellow/serviceable. Please provide references.
